Prognosis

The objective determination of the prognosis associated with brachycephalic syndrome from the data of veterinary literature is hindered by several difficulties. Most studies are retrospective and involve different populations of dogs in terms of breeds and conformations, which underwent several concurrent treatments in various combinations, performed at variable ages, using variable techniques and instruments.

Furthermore, many studies only rate the outcome in terms of improvement compared to the preoperative condition, but this preoperative condition is either ungraded, graded according to scales which are not consistent between preoperative and postoperative assessments, or described at the population level, which poorly reflects the severity of affected individuals (Clark and Sinibaldi, 1994; Davidson et al., 2001; Harvey, 1982; Lorinson et al., 1997; Pink et al., 2006; Riecks et al., 2007; Torrez and Hunt, 2006; Trostel and Frankel, 2010). Other studies use grading systems in an effort to objectively compare preoperative and postoperative clinical signs, but these scales are not consistent between studies (Brdecka et al., 2008; Dunié-Mérigot et al., 2010; Findji and Dupré, 2008; Huck et al., 2008). Lastly, follow-up times are also quite variable and in most cases the long-term follow-up is obtained by way of a questionnaire.

Keeping in mind these methodological limitations, it clearly appears that most dogs suffering from brachycephalic syndrome benefit from surgery in most recent studies. The proportion of dogs significantly improved by surgery is in excess of 90% (Dunié-Mérigot et al., 2010; Findji and Dupré, 2008; Poncet et al., 2006; Riecks et al. 2007), which is better than earlier reported (Harvey 1982; Lorinson et al., 1997). Similarly, perioperative mortality rates have improved from around 15% in earlier reports (Harvey, 1982; Lorinson et al., 1997) to less than 4% in more recent studies (Brdecka et al., 2008; Dunié-Mérigot et ...
