Bone marrow evaluation
Bone marrow aspiration cytology and core biopsy can be useful for diagnosis of MDS and leukemia. It is not uncommon to find large numbers of neoplastic cells present in bone marrow from animals with aleukemic leukemia (i.e. leukemia without detectable neoplastic cells in circulation). Thus, bone marrow evaluation is indicated in patients with persistent cytopenias or pancytopenia. Cytologic evaluation of cell morphology is best accomplished on aspirate samples, while biopsies are best for assessing marrow cellularity, myelofibrosis, and focal lesions.
Blast counts > 20% are consistent with acute leukemia, and careful assessment of morphology may provide evidence for neoplastic cell maturation and suggest a specific diagnosis (Fig. 2). However, reaching a specific diagnosis is problematic when blasts predominate. Blasts of different lineages often have a similar appearance, and lymphoblasts may or may not contain visible nucleoli (Fig. 6). Bone marrow samples may also be useful for additional testing such as cytochemical stains, immunophenotyping, and PCR (discussed below).
Neoplastic lymphocytes in CLL have normal morphology and don’t usually efface the marrow. Although lymphocytes are present in relatively low numbers in bone marrow from normal dogs (< 10%) and cats (< 20%), increased numbers may occur with chronic antigenic stimulation or immune-mediated disease.
